Understanding Cognitive Testing: What to Expect and When It's Recommended

What Is Cognitive Testing?

We offer two cognitive tests: One which we use for every Primary Care visit which include a brief, structured evaluations—often taking 5–30 minutes—designed to screen for cognitive impairment. Common tools include:

  • Montreal Cognitive Assessment (MoCA): A widely used 30-point test that assesses multiple domains, including memory recall, clock drawing, attention, and executive function. It's especially sensitive for detecting mild issues.

  • Mini-Mental State Examination (MMSE): A classic screening tool focusing on orientation, registration, attention, recall, and language.

  • Saint Louis University Mental Status (SLUMS): Another 30-point exam effective for identifying mild neurocognitive disorders, with tasks on orientation, memory, fluency, and visuospatial abilities.

These are typically quick office-based screenings, not full neuropsychological batteries (which can take hours and involve deeper analysis).

Cognitive testing is a screening step—it flags potential concerns but does not provide a definitive diagnosis on its own.


Then there is a Comprehensive Cognitive Test Appointment

Most sessions are straightforward and non-invasive:

  1. Preparation: No special prep is needed. Get a good night's sleep, eat normally, and bring glasses or hearing aids if required. Avoid alcohol or substances that could affect performance.

  2. The Process: You'll answer questions and complete simple tasks, such as:

    • Recalling a short list of words after a delay.

    • Drawing a clock or copying shapes.

    • Naming objects, performing calculations, or following instructions.

    • Tasks testing attention, like subtracting serial 7s or naming animals in a category.

  3. Duration and Setting: Usually 3 to 5 hours of multiple standardized testing

  4. Results: Scored immediately or soon after. A normal range varies by test and factors like age/education, but lower scores may prompt further evaluation (e.g., lab work, brain imaging, or specialist referral).

It's painless—no needles or machines involved—and many patients find it interesting. Results help guide next steps, not judge intelligence.

Who Should Consider Cognitive Testing?

Cognitive screening is recommended if you or a loved one notice changes, or as part of routine care for at-risk individuals. Common indicators include:

  • Increasing forgetfulness (e.g., repeating questions or misplacing items more often).

  • Difficulty with planning, problem-solving, or managing finances/medications.

  • Trouble finding words, following conversations, or concentrating.

  • Changes noted by family/friends.

  • Risk factors like age 65+, family history of dementia/Alzheimer's, history of stroke, diabetes, depression, or head injury.

Guidelines suggest alertness to early signs, especially in older adults. Medicare annual wellness visits often include basic cognitive checks, and proactive screening can support early intervention.

What Conditions Can Cognitive Testing Help Rule Out or Identify?

Cognitive tests screen for impairment but do not diagnose specific diseases alone. They help:

  • Screen for signs of neurodegenerative conditions like Alzheimer's disease, vascular dementia, or Lewy body dementia.

  • Rule out or differentiate from reversible causes, such as medication side effects, vitamin deficiencies (e.g., B12), thyroid issues, depression ("pseudodementia"), or sleep disorders.

  • Identify when further testing (e.g., MRI, bloodwork, or detailed neuropsych eval) is needed to confirm or exclude conditions.
