At PCP-Health, PLLC, we strive to provide high-quality primary care, functional medicine, and psychiatric services. This Refund Policy outlines our approach to refunds for services rendered by our board-certified Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP) with autonomous practice authority in Florida. By utilizing our services, you agree to the terms below.
1. Non-Refundable Services
Services provided (e.g., consultations, assessments, telehealth visits) are non-refundable once rendered, as they involve professional expertise and time commitment.
No refunds will be issued for services covered by insurance (e.g., Medicare, Medicaid, private insurers) unless denied by the payer, in which case you may request a review.
2. Cancellation/No-Show Fees
A $50 fee may apply to cancellations with less than 24-hour notice or no-shows, unless waived for documented emergencies (e.g., medical or family crisis). This fee is non-refundable but may be credited toward a future visit at our discretion.
3. Prepaid Services or Packages
Prepaid service packages (e.g., wellness plans) are non-refundable after initiation but may be prorated or transferred to another service if requested within 7 days of purchase, subject to a 10% administrative fee.
Unused portions of packages expire 12 months from purchase unless otherwise stated.
4. Exceptions
Refunds may be considered on a case-by-case basis for extraordinary circumstances (e.g., severe illness preventing attendance), with written documentation required. Decisions are final.
Refunds, if approved, will be processed within 30 days via the original payment method, minus any applicable fees.
5. Disputed Charges
If you dispute a charge, contact our office within 30 days with details. We will investigate and respond within 14 days. Unresolved disputes may be referred to your insurance provider or collection agency.
6. No Warranty
Services are provided “as is” without warranties beyond professional standards of care. PCP-Health, PLLC is not liable for indirect damages related to refunds.
7. Governing Law This policy is governed by Florida law, with disputes resolved in [Charlotte, FL] courts.
8. Modifications We may update this policy with notice posted on our website or in-office. Continued use of services constitutes acceptance.
