PCP-Health, a Professional Limited Liability Company (PLLC) based in Florida, providing primary care, functional medicine, and psychiatric services by a board-certified Family Nurse Practitioner (FNP) with autonomous practice authority and Psychiatric-Mental Health Nurse Practitioner (PMHNP) with Psychiatrist agreement. These Terms and Conditions govern your use of our services, website, and related platforms. By accessing or using our services, you agree to be bound by these terms. If you do not agree, please refrain from using our services.
1. Services PCP-Health, PLLC offers integrative healthcare services, including primary care, mental health assessments, and functional medicine consultations. Services are provided in-person or via telehealth, subject to availability and your consent. All services are delivered within the scope of practice defined by the Florida Board of Nursing. We reserve the right to refuse or discontinue services for non-compliance, safety, or legal reasons.
2. Appointments and Cancellations

Appointments must be scheduled via our website or phone call.

A 24-hour cancellation notice is required, or a fee (up to $50) may apply, unless waived for emergencies.

Late arrivals may result in rescheduling at our discretion.
3. Payment and Insurance

Fees are based on a fee schedule provided at intake. Payment is due at the time of service via cash, credit card, or approved payment platforms.

We accept Medicare, and private insurance where credentialed. You are responsible for copays, deductibles, or non-covered services.

Unpaid balances may incur interest (1.5% monthly) and collection actions after 60 days.
4. Use of Website and Technology

Our website and integrations (e.g., Headway, Practice Better, Ai) are for informational and scheduling purposes. Content is not medical advice; consult a provider.

You agree not to misuse our platforms (e.g., unauthorized access, data scraping). Violations may lead to account termination.

We use cookies and analytics for functionality and improvement, per our Privacy Policy.
5. Privacy and Confidentiality

Your protected health information (PHI) is governed by our Notice of Privacy Practices, compliant with HIPAA and Florida law. We may share PHI for treatment, payment, or operations, with your authorization for other uses (e.g., marketing). Contact our Privacy Officer for concerns.
6. Limitation of Liability

PCP-Health, PLLC is not liable for indirect to the extent permitted by law. Our liability is limited to the fees paid for the service in question.
7. Termination of Services

We may terminate your care for non-payment, disruptive behavior, or legal/ethical breaches, with notice where feasible. You may discontinue services with written notice.
8. Students

Students can be present during consult for learning & experience.
9. Governing Law

These terms are governed by Florida law. Disputes will be resolved in [Charlotte FL] courts.
